CoCare is a documentation system designed to connect aged-care workers, aged-care residences, and the families of residents.

The system was designed based on the CoCare model, which ensures that (1) the effort spent in the documentation process was reduced and (2) the hidden carework becomes visible

CoCare Record, an AI-assisted documentation tool, allows aged care workers to document on the go with ease, while guaranteeing the security and accuracy of information.

With CoCareConnect, the family members will have access to daily updates of their family members in aged care facilities. CoCareChatbot can be accessed for common queries family members have regarding residences.

CoCare Documentation System

This project, CoCare, responds to the brief “Making Invisible Carework Visible” by addressing communication gaps between aged care workers and families. Through interviews and research, our team identified that aged care nurses spend excessive time documenting care, while families often feel uninformed and anxious. To address this, CoCare introduces a streamlined documenting tool and a family-facing app that provides timely, accessible updates about residents’ wellbeing. The system reduces nurses’ administrative burden, supports multilingual communication, and strengthens trust between families and staff. The CoCare system was designed to contain multiple parts that address the needs of different stakeholders: the aged care workers, registered nurses, family members and aged care residents. By making everyday care activities visible, CoCare helps maintain emotional connection, relieves stress, and reinforces the value of caregivers’ essential but often overlooked work.
